Our Team
The Carrollton team is comprised of compassionate and caring clinicians, including physical and occupational therapists, speech pathologists, RNs, LPNs, and CNAs, all of whom work under the direction of your private physician. Our entire staff of dedicated professionals treat all patients and residents holistically, engaging them physically, mentally and spiritually to help maintain physical function, self-esteem and emotional well-being.

Janice Fisher, LNHA
Administrator
I have been working in long term care for 21 years—12 of these as a licensed nursing home administrator. My career began in 1999 when I held a receptionist/medical records position. Over the course of eight years I worked in many different roles in the skilled nursing home setting, which prompted me to become an administrator. I subsequently obtained my license in 2007.
Being a positive influence in the lives of the residents, their families, and staff is very important to me. Holding a leadership role allows me to guide others in providing quality care and support in the nursing center setting. The relationships that are built with the residents are invaluable.
I am proud of my years of service as Administrator of Carrollton Nursing and Rehabilitation Center and strive to make a difference every day.
Pamela Johnson BSN, R.N.
Director of Nursing
I am currently the Director of Nursing at Carrollton Nursing and Rehabilitation. I have been a nurse for 20 years. I started my career in long-term care in 2000. I have worked in several health care settings, i.e., dialysis, hospice, and rehab. It is my love and passion for the elderly that led me back to geriatric nursing. I left bedside in 2017 and ventured into management. My goal is to positively impact everyone I meet and make a difference in the lives of my residents.
Jami Houston
Business Office Manager
I am a seasoned Business Office Manager with over 15 years of experience with all administrative tasks. Having held multiple position in the medical and business fields, I have tailored the skills to meet the needs to various clients. I am an accomplished professional with diverse management experience, including payroll/HR and accounts receivable. I have been employed with Carrollton Nursing & Rehab Center since 2002 and have been recognized numerous times with an “Outstanding Accounts Receivable Award.” I graduated from the University of West Georgia in 2001 with a B.A. in business management and a minor in accounting.
Irene Reynolds
Social Services
I began my work career at Carrollton Nursing and Rehab as a nursing assistant in the early 70s while attending college. After I obtained my B.S. in sociology and psychology, I became the social worker and have been employed here for 43 years. The joy of working here has always been seeing the difference that is made in our residents’ lives.
Courtney Melville
Admissions Director
I have worked in long-term care for over seven years in various positions. I started out as a CNA, but I have worked as a staffing coordinator, central supply and transport and business office assistant.
The residents we help take care of is what makes long-term care so special. We get to guide them through a time that some have never experienced while also learning about their journey through life. Every resident and every story is unique, but we treat all of our residents the same—like family.
Greta Ayers
Activities Director
I have been employed at Carrollton Nursing and Rehab for the past 24 years. I started my employment here as a CNA and within six months was offered to be part of the bath team. I really enjoyed my time as a CNA/bath team member, and I loved how it made me feel like I made a difference in all of my residents’ lives. After a few years, I worked in central supply and later I also worked as the staffing coordinator. I have also worked as a restorative aide and in central supply. From there I became the activities director for the facility, and I have enjoyed every day of making our residents smile. It makes my heart happy to see the residents happy and enjoying themselves through the years at all of our special events and activities.
Cynthia Miller, SLP-CCC
Rehab Director
I have been the Director of Rehabilitation at Carrollton Nursing and Rehabilitation center for over five years. I have been a Speech-Language Pathologist for six years. I am a graduate of the University of West Georgia with a bachelor’s degree in communication sciences and disorders, with a minor in business administration, and a master’s degree in communication sciences and disorders. I am certified and trained to utilize VitalStim Plus to treat swallowing disorders. I actively mentor clinical fellows for speech-language pathology and provide training/supervision of speech-language pathology students from universities across the state.
I am passionate about working with the geriatric population because my priority is to ensure an exceptional quality of life is maintained during my patients’ stay with us at Carrollton Nursing and Rehabilitation Center. I enjoy being able to care for and provide a loving environment for our residents during their stay with us.
I am fortunate to be the handler for our facility therapy dog, Maizey. Maizey is a Goldendoodle that we have had for over a year and has grown to be a part of my work and home family.
Saulo Franca
Environmental Services Director
After being involved with sports most of my life, I changed pace and embraced a career in Environmental Services in 2014. Like sports, I like the fast pace challenges and the need to adapt to an ever-changing environment. I also enjoy providing a clean and safe space for our residents to live and play during some of their most important years. My wife and I are dedicated to serving others in healthcare and our children are along for the ride with servants’ hearts.
Vedra Parham
Food Services Director
I began my employment for Carrollton Nursing and Rehab in Dietary Services in 2008.I then became the Assistant Food Director and was in that position for 8 years. I am now the Food Services Director and have been since May 2019. I really enjoy being able to make sure the residents can enjoy the quality of foods from the dietary department.
Collin Clarke
Maintenance Director
I have 28 years of experience in the maintenance industry and have many certifications such as Carpentry Advanced, Electricity Advanced, Masonry Advanced, Plumbing Advanced, Small Engines Advanced, HVAC Universal, OSHA Universal, CPO, and Serve Safe. I enjoy working at Carrollton Nursing and Rehab and interacting with the residents. Sometimes the smallest act of kindness can make someone’s day.
Melissa Stephens
Payroll Coordinator
I have 10 years of service at Carrollton Nursing & Rehab. Nine of these as a Payroll Coordinator. I really enjoy working with numbers. My favorite part of clinicals was the long-term care rotation. With that said, when I had the opportunity to work at a nursing center I accepted it to be able to work in LTC in some way.
